The BSA has just simplified the annual health and medical record by shortening Part A (Informed Consent, Release Agreement, and Authorization) and requiring only the most pertinent medical information under Part B (General Information/Health History).  Part C (Pre-Participation Physical) has been reduced to one page with expanded sections for explanations of allergies while the Supplemental Risk Advisory (former Part D) has been shortened and is location specific.
